Parent Checklist for Enrollment

In order to begin the process of enrolling your child in the Second Street Learning Center you will need to present the following documentation to the Intake Department, located in the second floor Administrative Office:

• Copy of Child’s Most Recent Physical
• Copy of Child’s Immunization Records

Upon receipt we will enter this information into our Well Care Tracking System to ensure that all necessary information is up to date. If the information is not up to date you will be advised as to what information must be updated prior to beginning the enrollment process.

When all information is current an appointment will be scheduled for your Intake. There is a non-refundable Registration Fee payable at the time of your Intake. If you do not present the appropriate fee as listed below your Intake appointment will not proceed:

• Funded Childcare – $15.00 Registration Fee
• Private Childcare – $30.00 Registration Fee

At the time of your Intake appointment you need to bring the following:

• Child/Children’s Social Security Card
• Child/Children’s Medical Card
• Your Photo I.D.

Please be advised that your child will not be admitted to the Second Street Learning Center until all proper documentation is in place. If you have any questions, please feel free to contact our Intake Department at 610-374-4696, ext. 242.

 Con el fin de iniciar el proceso de matricular a su hijo en el Centro de aprendizaje tendrá que presentar la siguiente documentación al Departamento de Admisión que esta en el segundo piso de la Oficina Administrativa:

• Copia del fisico más reciente del niño
• Copia del Registro de Vacunas del niño

Cuando recibimos esta información se introducirá en el Sistema de Seguimiento para asegurar que toda la información necesaria está al día. Si la información no es corriente  nos le informaremos qué información se debe actualizar antes de comenzar el proceso de inscripción.

Cuando toda la información este actualizada una cita será programada para su admisión. Hay una no-reembolsable Cuota de inscripción para pagar en el momento de su admisión. Si no se presentan con la tarifa correspondiente que figuran a continuación de la cita de admisión no procederá:

• Guardería Financiada – Cuesta de inscripción $15.00
• Guardería Privada – Cuesta de inscripción $30.00

En el momento de su cita de admisión tiene que traer lo siguiente:

• La Tarjeta de Seguro Social del niño/niños
• La Tarjeta Medica del niño/niños 
• Su foto de identificación
